Immunization Records
At SUNY Cobleskill Ag & Tech, email is the official means of communication for the campus, so it is imperative that each student activate and use their Cobleskill network account and email service. In order to submit your medical history information, and immunization information, you will need to activate your Cobleskill email account. Click here and follow the directions. Once you have established your email account, you will then be able to proceed with the directions below. All future correspondence will be through your Cobleskill email.
Students who attend Cobleskill Ag & Tech and are enrolled for 6 or more credits must submit the Tuberculosis Screening Questionnaire and Immunization information.
Medical Records Requirement Due Dates:
Fall Semester- August 1st - Spring Semester- January 3rd
Prior to arrival to campus, all students registered for 6 credit hours or more MUST submit the following:***
- Verification of MMR immunity
- Meningococcal Meningitis Vaccination Date(s) or Refusal Letter
- Completed Tuberculosis Questionnaire and testing (if applicable)
***Non-compliance with the above requirements may result in dismissal from Cobleskill Ag & Tech
1. Login to Patient Portal
Go to the Patient Portal and follow the directions to register. Once you have registered, you will receive a link in your Cobleskill e-mail account that will prompt you to create and verify your new password. This will give you access to your Patient Portal account where your medical history information and immunization information is submitted.
- Go to www.cobleskill.edu
- Hover over "Campus Life" in the orange navigation bar.
- Click on "Health & Counseling"
- Click on the "Patient Portal Icon" found on the right hand side of Wellness Center home page.
- Click "Register"
- Enter the following:
- Name (exactly like it appears in Banner)
- 800#
- Cobleskill email (Will not accept any other email)
- Click on “Submit”. A box will pop up that says an email (from the Wellness Center) has been sent to your Cobleskill email account
- Open email and copy the hyper link in the email (this will prompt you to create and confirm password)
- Click on "Submit" (once a password is created it will take the student directly into their account. If it doesn't, something was done incorrectly and you will need to go back to the email and copy and paste the link again)
2. Submit Your Required Medical History Information
To complete and submit your medical history information, please select "Forms" and complete the "Medical History Form".
1. In the Patient Portal, click on "Forms"
2. Select "Medical History Form"
3. Fill out "Medical History Form"
3. Click on "Submit" (if the form does not disappear it means that you have missed a required question)
3. Submit Your Required Immunization Information
All immunizations must be verified by Wellness Center staff through one of the following methods.
- A copy of your immunization record can be uploaded to your patient portal account *
- A copy of your immunization record can be faxed to 518-255-5819*
- A copy of your immunization may be emailed to wellnesscenter@cobleskill.edu*
- A copy of your immunization record can be mailed to:*

* A copy of your immunization history may be obtained from your graduating high school or your family physician.
Your Health and Immunization Information will NOT be considered complete until a copy of your immunization record is received and verified by Wellness Center staff. Failure to submit your record, may delay your registration process.
NEW YORK STATE LAW: MMR REQUIREMENTS
Records may be located at your previous college, high school, or family doctor’s office.
In accordance with NYS Law any student born after January 1, 1957, must provide proof of immunity to measles, mumps, and rubella, this requirement may be met by the following: proof of disease, antibody titers, or two {2} measles, one {1} mumps, and one {1} rubella vaccine given after the student’s first birthday.
NEW YORK STATE LAW: MENINGOCOCCAL MENINGITIS VACCINE
The Center for Disease Control Advisory committee on Immunization Practices (ACIP) recently issued a new recommendation for college students registered for 6 credit hours or more: Students, especially those living in residence halls, should consider vaccination to reduce their risk for meningococcal disease.
New York State Public Health Law 2167 requires:
A response to receipt of meningococcal disease and vaccine information signed by the student or student’s parent or guardian (if under the age of 18)
AND EITHER
A record of at least 1 dose of Meningococcal ACWY vaccine within the past 5 years or a complete 2 or 3 dose series of MenB
OR
An acknowledgement of meningococcal disease risks and refusal of meningococcal vaccination response form signed by the student or student’s parent or guardian (if under the age of 18).
Cobleskill Ag & Tech does not offer the meningococcal meningitis vaccine. Students should contact their private health care provider for vaccination. Click the following link for further information on Meningococcal Disease:

TUBERCULOSIS SCREENING REQUIREMENT
As part of Cobleskill Ag & Tech’s health requirements, all students at high risk for tuberculosis must have TB testing completed within 6 months prior to the start of classes.
If you meet any of the following criteria, testing is required:
- Born in, lived in, or have had frequent or prolonged visits (for a period of one month or more) to a country where TB is common
- Close contact with someone diagnosed with active TB
- Have been a resident and/or employee of high-risk congregate settings (such as correctional facilities, long-term facilities, and homeless shelters)
- Occupational exposure (volunteer or healthcare worker who provided care to patients at increased risk for active TB)
- Have a history of or are currently using illegal, injectable drugs
- Weakened immune system (HIV, certain medications, etc.)
- Abnormal chest x-ray suggesting previous TB

Currently, SUNY students are strongly encouraged to stay up to date with their COVID-19 vaccinations. While COVID-19 vaccinations are not required at this time, SUNY reserves the authority to change this policy at any time because of changes in local conditions or due to requirements imposed by Federal, State, or Local authorities.
